Understanding Claravelle

What is care management?

Care management helps individuals and families navigate healthcare when there are too many moving pieces to manage alone.

Claravelle helps organize the bigger picture—providers, appointments, medications, hospital or rehabilitation transitions, transportation needs, follow-up tasks, communication, and other care-related concerns.

Think of us as the people helping make sure important details don't get lost between appointments, providers, facilities, and family members.

Is Claravelle a home health agency?

No. Claravelle Care Management is a private, nurse-led care management service. We do not provide hands-on home health services such as bathing, wound care, injections, medication administration, or other skilled nursing treatments in the home.

Instead, we help coordinate and manage the care surrounding those services.

For example, if your parent needs home health after leaving the hospital, we can help determine whether it was ordered, assist with coordinating the agency, track unresolved issues, and help communicate concerns to the appropriate healthcare provider.

Who can benefit from Claravelle?

Our services may be helpful for:

What exactly can Claravelle help me with?

Depending on your needs and service plan, Claravelle can assist with things such as:

Healthcare coordination — helping keep providers, appointments, referrals, and services organized.

Medication review and reconciliation — helping identify discrepancies and questions that may need to be clarified with your healthcare providers.

Hospital-to-home transitions — reviewing discharge needs and helping coordinate follow-up appointments, prescriptions, equipment, services, and unresolved discharge items.

Appointment preparation and advocacy — helping you prepare questions and information for appointments and, when arranged, providing in-person appointment advocacy.

Transportation coordination — helping identify and arrange appropriate transportation resources.

Family communication — with the client's authorization, helping keep designated family members informed.

Care planning — identifying current needs, barriers, priorities, and gaps and developing an individualized Care Roadmap.

Can you talk to my doctors for me?

With appropriate authorization, Claravelle can communicate and coordinate with healthcare providers on your behalf.

We may help relay information, clarify care instructions, follow up on outstanding items, coordinate between providers, or help make sure important concerns reach the appropriate member of your healthcare team.

Claravelle does not replace your physician or other treating providers, and medical decisions remain between you and your healthcare providers.

Can you attend a doctor's appointment with me?

Yes, when available and arranged in advance, Claravelle offers in-person appointment advocacy.

We can help you prepare beforehand, organize questions and concerns, accompany you to the appointment, take notes, help you understand the follow-up plan, and assist with coordinating the next steps afterward.

This can be especially helpful when appointments involve a lot of information or when family members cannot attend.

Can you help after someone comes home from the hospital or rehabilitation?

Absolutely. In fact, transitions are one of the times when care coordination can be especially valuable.

Claravelle can review the discharge plan, help track medications and prescriptions, follow-up appointments, medical equipment and services, transportation needs, and unresolved discharge items.

Our goal is to help answer one very important question: “Now that we're home, what needs to happen next?”

Working With Families

I live outside Citrus County, but my parent lives there. Can Claravelle help?

Yes. This is an excellent situation for private care management.

With your parent's permission and the appropriate authorizations, Claravelle can serve as a local healthcare coordination resource while keeping authorized family members informed.

For adult children living elsewhere, having someone local who understands healthcare and can help follow up on the moving pieces can provide valuable support.

Will you communicate with family members?

Yes, when authorized by the client.

Privacy is extremely important to us. Before discussing a client's healthcare information with family members or other individuals, Claravelle obtains the appropriate authorization.

The client remains at the center of the care-management relationship.

Getting Started & Paying for Services

Do you accept insurance or Medicare?

Claravelle is a private-pay care management service. We do not bill Medicare or health insurance for our services.

This allows our services to be based on the client's individual needs rather than an insurance company's coverage requirements.

How much does care management cost?

Claravelle offers several levels of service depending on the amount of assistance needed.

Services range from one-time assessments and transition support to ongoing monthly care management and hourly professional services.

Current services and pricing are available on our Services page. We can also help you determine which option best fits your needs during your complimentary Discovery Call.

Do I have to sign up for monthly services?

No.

Some clients need ongoing care management, while others may only need help with a specific situation—such as a comprehensive assessment, hospital-to-home transition, or appointment advocacy.

We'll discuss what's happening and help determine which service makes the most sense.

What happens during the free Discovery Call?

The complimentary Discovery Call is a brief introductory conversation—up to 15 minutes—to learn what's happening, what kind of help you're looking for, and whether Claravelle appears to be an appropriate fit.

It is not a comprehensive clinical assessment, and there is no obligation to enroll in services afterward.

What happens if I decide to become a client?

We'll guide you through enrollment and obtain the information and authorizations needed to begin.

Depending on the service selected, the process may include a comprehensive care assessment, review of current providers and medications, identification of immediate concerns and care gaps, and development of your personalized Care Roadmap.

From there, Claravelle begins helping coordinate the agreed-upon pieces of your care.

Important Boundaries

Can I call Claravelle in an emergency?

Claravelle is not an emergency medical service and does not provide emergency response or 24-hour clinical monitoring.

If you believe you are experiencing a medical emergency, call 911 or seek emergency medical care.

Does Claravelle diagnose conditions or prescribe medications?

No.

Claravelle provides care management, education, advocacy, and coordination within our professional scope. We do not replace your physician, diagnose medical conditions, prescribe medications, or independently change your treatment plan.

If we identify a concern or discrepancy, we can help communicate it to the appropriate healthcare provider.

Does Claravelle provide transportation?

Claravelle can help coordinate transportation, but we are not a transportation company.

We can assist with identifying appropriate transportation resources and arranging services based on the client's needs and available options.
