Includes: Up to 15 minutes. This is a fit conversation, not a comprehensive clinical assessment.

A structured review of medical history, diagnoses, providers, medications, recent healthcare utilization, support system, functional context, barriers, current services, upcoming needs, and care-coordination gaps. Claravelle develops a written Care Roadmap based on the assessment.

Includes: Up to 90 minutes, written Care Roadmap, and follow-up.

Focused hospital-to-home or rehabilitation-to-home transition management. Claravelle may coordinate with the discharge team, review the discharge plan, track prescriptions/equipment/services, help coordinate appointments and transportation, follow unresolved discharge items, and provide authorized family updates.

Includes: Up to 8 professional hours over 14 days. Additional time is billed at the applicable hourly rate.

For clients who need consistent oversight and coordination without intensive weekly management.

Includes: Up to 2 professional hours per month, scheduled check-in, and care-coordination activity within the included time.

For clients with more moving parts who need more active coordination, communication, and follow-through.

Includes: Up to 4 professional hours per month with more active care-management coordination.
